Why do lentigines occur?

Solar lentigo is a flat, brown pigmented lesion that primarily appears due to UV exposure and skin aging.

This refers to a condition where melanin cells are excessively activated, leading to an over-accumulation of pigment in specific areas.

Repeated exposure to UV radiation continuously triggers melanin production,

and as one ages, the skin's regenerative capacity and pigment metabolism decline, which can gradually lead to darker color and wider spread.

They are typically observed on the face, back of the hands, and arms, characterized by distinct borders and a dark brown color.

 

Location of Lentigo Distribution

Lentigines (solar lentigines) primarily appear confined to the epidermis of the skin.

Specifically, melanocytes in the basal layer become excessively activated due to UV stimulation,

resulting in the localized accumulation of melanin pigment.

Histologically, lentigines do not penetrate the dermis but are characterized by an excessive accumulation of melanin within the epidermis,

and their borders with surrounding normal skin tissue are relatively clear.

Due to these characteristics, they tend to respond well to treatments targeting the epidermal layer, such as Q-switched 532nm lasers.

Solar lentigo, which primarily appears in the epidermal layer, is distinct from melasma or complex pigmented lesions that affect even the dermal layer.

 

532nm Laser Wavelength

Melanin pigment particularly absorbs the 532nm wavelength well. This green wavelength primarily targets and improves pigmented lesions located in the epidermis, the outermost layer of the skin.

The 532nm laser typically utilizes a Q-switched mode. This focuses and delivers high-power energy in very short nanosecond pulses. As a result, it can precisely destroy only the pigment particles while minimizing damage to surrounding tissues.

When short pulses are selectively absorbed by melanin, they are converted into thermal energy. Simultaneously, photomechanical shock occurs, causing the melanin particles to break into tiny fragments.

Some of these finely fragmented pigments naturally shed as scabs, while others are processed by the body's macrophages and gradually eliminated through the lymphatic system.

As the destroyed pigment gradually disappears, you can expect the solar lentigo to become visibly lighter or generally fade.
